MEMO: Out of Jurisdiction Placement Review

Tikinagan Child & Family Services will be holding Mamow case conferences in August, regarding youth who are placed in OPRs.

Mamow Obiki-ahwahsoowin case conferences will become a regular occurrence with our youth placed in outside purchased resources. This will allow workers to better know and understand their youth’s complex needs, the services that are being provided to them, educational needs that they are receiving, extra-curricular activities that they may be participating in. The unit is to have a plan for repatriation/reunification. The agency wants to see progress with our youth and ensure that the goals/plan is in place to have them return to our catchment area.

It is the unit’s responsibility to have case conference notes prepared prior to each meeting with a full update and focusing on the future planning for our children.

Who should be at these meetings are as follows, Service Manager, Family Service and Child Care Direct Services Supervisor, Family Service Worker and Child Care Worker. The expectation is that all members of the youth’s team attend these conferences. Invitations will be sent out in August.

Prior to the scheduled meeting, please prepare packages and submit to Julianne Chapman by August 12, 2022.

  • The youth’s legal status
  • ISA, RSA or SRA – why is the youth on it, is a reduction or increase warranted
  • SOR’s that have been submitted in the past 3 months
  • Rights and Responsibilities
  • Last time youth was seen by their Tikinagan CFS Child Care Worker (PIV)
  • The last Tikinagan Child & Family Services Plan of Care
  • Reunification/ Repatriation Planning for the youth
  • The last OPR Plan of Care
  • Access to extended family and community
  • Update on how the youth’s family is progressing towards their service planning
